CMS-1500 V1.09 Software Operating Instructions

SOFTWARE INSTALLATION: To install the software, click the link below. The installation program will start and on the installation menu choose NEXT then NEXT again, and then FINISH.  The Main Menu for the blank CMS-1500 form will appear on your computer ready to use and an icon for the CMS-1500 program will be added to your desktop for future use.

MENUS: It is recommended that you review the six menu items found along the top of the main menu page; File, Edit, Field, MENU, Text File and Template. 

SAVING CLAIMS: To save a claim, go to the menu item at the top left and click File and Save As, and name the file and click Save.

PRINTING:  SmartForms CMS-1500 V1.09 will print claims on preprinted forms or on plain paper, generating the form and content. To print a claim, click the top left menu item File, and Print Menu and choose either Print On Form or Print on Plain Paper.  The claim will appear in a print preview and next you will click the Printer icon and Close.

ALIGNMENT:  The alignment process is a self-serve process and easily accomplished.  From the Main Menu click New Patient.  Complete a portion of the form and print on a red preprinted form.  (Do not use copies of the red pre-printed forms or tractor feed forms.)  Look at the form you printed and if all the information falls correctly in the boxes, the alignment process is complete.  IF the alignment needs to be adjusted, click the File menu item in the top left corner, next click Print Menu, next click Move.  You will see all the field boxes in blue and you will make the adjustment by using the arrow keys on your keyboard.  For each click on an arrow key the form will adjust in that direction one quarter of a space.

This is a stand alone product and does not need Word or Excel to operate.
