Step 1.

Click the link below and follow the simple instructions to save the demo version of SmartForms C-4 software on your computer.

Installation

When the SmartForms installation begins, click “Save File” in the window that appears.

When the download is complete, double click the file name (demo_C_4(3).exe) and when the box appears “Open Executable File?” Click OK to launch the demo and you will be taken to the Main Menu page of SmartForms C-4 Software.

Using the Software

Click on New Patient and a blank C-4 form is ready for information. Fill in the form and click “File” and “Save As” and name the file. You have now successfully completed the new C-4.0 form and are ready to print. Note: The demo program will only print page 1.
